All new families must agree to the K5 Parent Agreement in order to participate in BAHA program.

Medical Waiver

Each year ALL families must complete a new Medical and Liability waiver to have on file for the current school year. Returning families will have this in their registration packets. New families, please complete this form and turn it in at your soonest convenience.
